In order to get you started using our new online system, you will need to be assigned a PIN (personal identification number) and a password. To receive your PIN, please fill out this form completely, and read carefully and agree to the terms and conditions for using our system.

Online Scheduling Terms and Conditions

In consideration for the confidential PIN number that you will be receiving and the ability to use our online scheduling system, you the customer, [blank], must agree to the following: To provide us with a valid credit card number with expiration date and your signature for us to keep on file, and authorize Fran Coy's Salon to charge your credit card for up to 50% of any missed appointments that were scheduled through the online scheduling system, and were not cancelled at least 48 hours before the appointment.

In order to cancel or change any of the appointments, that you have scheduled online, you must call the salon during normal business hours, and cancel through one of our receptionist. If the services that you schedule are not cancelled within 48-hours of the appointment, than you the customer, [blank], agree to pay 50% of the total costs of the services using the credit card number that you will be providing later.
